Service Description

This is a tele-health appointment with the BridgeCare team following an inpatient or observation discharge. (ER or urgent cares do not qualify) We will review your recent treatment. We can send refills, sign orders, answer questions, talk to your health team and launch you to better recovery. NOTE: We will call you to confirm your appointment. If you are unable to confirm, your appointment may be canceled.
