To refer a patient to Physicians on Call (PoC) Patient Monitoring services, check the following:

 

Step 1:
Patient must meet all thee criteria below.
 

- Patient has a chronic condition.

- Patient is interested in improving chronic conditions.

- Patient has Medicare or Medicare advantage.

Step 2:
Complete and fax

<--  Physicians on Call Referral Form to BridgeCare.

​

 

BridgeCare will contact patient, verify eligibility and participation in the program, and mail monitoring device. Patient should receive their device within a week and be ready to use it right out of the box.
