BridgeCare's patient monitoring at home is a covered benefit for Medicare beneficiaries and covers all equipment and services provided under BridgeCare. An individual covered by Medicare and has a chronic disease listed below is eligible for home monitoring devices. Accepting diagnosis are expanding but at present a patient typically has one or more of the following diagnosis:
BridgeCare’s Physicians on Call works alongside primary care and endocrinology clinics to manage chronic care conditions.
We provide diabetic patients dedicated to improving/managing their health with:

4G-enabled home monitoring equipment to check blood sugars (no wifi required)

All quarterly test supplies (lancets, strips, batteries)

Daily oversight from a BridgeCare physician

A care team that will contact patients if their values are outside normal

Education and health coaching to help avoid hospitalizations and poor outcomes

Additional guidance to follow up to assist PCP with appointment and medication compliance

QUALIFYING CONDITIONS:
Any Medicare patient with insulin dependent diabetes is eligible to participate in our remote monitoring program.

Is your patient a good candidate?
Our PMH program is best suited to patients who are interested in taking an active role in managing their chronic conditions and interested in using their devices at least once every other day.
