New Patient Registration and Consent



BridgeCare is honored to be considered a part of your care team.
Our goal is to help make your continued recovery smoother and ea
sier for you.

As a discharge clinic, we will be serving as your temporary primary care provider.  This includes refilling medications, managing orders, providing guidance, and much more throughout your ongoing recovery at home.

Our comprehensive services are eligible for insurance coverage, contingent on our ability to schedule an onboarding visit promptly. There's a limited timeframe within which we must schedule your initial video appointment. Insurance allows us  13 days from your discharge to have a video visit with you before our service eligibility ends.

Once you're successfully onboarded as a new patient, you will have all of our services for up to 60 days.

To streamline the onboarding process, we have a few essential questions to ask. Kindly respond to the best of your ability. Once received, we will call you to schedule your first appointment.
