60 days of post-discharge care in the comfort of a patient's home. BridgeCare also offers build support and training regarding discharge services, value-based care, and more.
BridgeCare's specialized doctors are trained in both inpatient and outpatient medicine. They know what each side can offer and where gaps in care typically fall. This allows BridgeCare the opportunity to offer a specific array of services to fill this gap and improve outcomes.
We provide comprehensive medical assistance during our visits until patients are able to connect with their primary care physician, whether it's establishing new care or re-establishing previous care. Additionally, we successfully manage acuity-based needs to allow BridgeCares to achieve a hospital and ER re-admission rate of less than 2%.

THE BRIDGECARE DIFFERENCE

BridgeCare providers manage prescription refills, oversee IV antibiotics, and do a thorough medication reconciliation with every transitional care appointment.

SUPERVISION OF HOME CARE
BridgeCare works hand-in-hand with home care companies. We conduct face-to-face visits to allow home care to help patients right away.
FLEXIBLE SCHEDULING
Our telehealth model allows patients to do visits at their own convenience. This includes weekends and evenings, increasing follow-up rates and preventing the need for travel.
IV ANTIBIOTIC MANAGMENT
Our physicians will order home labs for patients to safely finish their IV antibiotic therapy at home. This allows patients to also be discharged home earlier and reduce their overall length of stay and cost.
WOUND GUIDANCE
With the help of virtual wound specialists, our team can help manage wounds by providing guidance, orders, and referrals.
