AMBULATORY COVID-19 TREATMENT (ACT) Program
At this time A.C.T patients are being accepted as Hospital at Home Referrals.
Custom A.C.T Care plans are available based on the availability
of the patient's local area resources.
BridgeCare's ACT program is intended to assist patients and hospitals throughout Michigan with COVID management. BridgeCare's ACT physicians can assist patients who are ready for discharge but require continued oxygen and symptom management at home.
ACT program accepts any Michigan health insurance.
Patients must be over the age of twelve to qualify.
