We can assist in improving diabetic hypertension CHF and wait for goals that translate to improve quality-based payments for the clinic.
Patients who are unable to see their primary care following an inpatient discharge can also be assisted through BridgeCare’s Hospital to Home Services.
Through referral for Physicians on Call, we can provide this additional assistance to both improve MIPS and quality-based scores for outpatient clinics throughout Michigan.
