By requesting to book, you will be prompted to sign a copy of our consent forms and select a date and time for your telehealth appointment. Case managers and family members can schedule a patient's first visit on their behalf, but a referral must have been accepted by BridgeCare in order to confirm the appointment.

Patients must be able to be seen for their first telehealth visit within the first 7 days of discharge to be eligible for our services.
